The best approach to our evaluations, clinically, is to present information from 3 sources: test scores and interpretation, family reports, and clinical observations.
Your format prevents this clarity, and does not allow me to create my own categories.
As a result, if I use your format, I have to clarify my information source every sentence or paragraph.
Also, by front-loading test scores, it creates the impression that they are the primary means of assessment, and that everything following is commentary on them, which is not correct.
Any way we will ever be able to create our own headings/sections?

Our auditors need to be able to see a set of specific objective info, immediately on opening a SOAP note. We are currently recording this data under Subjective.
This is problematic for therapists bc it's not correct and belongs with Objective info, and also for the auditors, who don't want to read or search through lengthy notes to find this set of statements.
Can you please help us by adding an empty box, or one labeled Notes or Format or Background at the very beginning of the SOAP note template?
